Ongoing Care was established in 2020, with a commitment to providing quality, person-centred disability and community support services.

Since 2021, we have also been providing Aged Care services, supporting older Australians to remain safe, independent and connected within their homes and communities.

Today, Ongoing Care provides NDIS and Aged Care services across Sydney and Melbourne, supporting people with disability, older Australians, families and carers with services tailored to their individual needs, goals and circumstances.

We believe every person should have choice, control, dignity and the opportunity to live as independently as possible. Our approach focuses on understanding each person’s individual needs, preferences, culture, communication requirements and personal goals.

We work collaboratively with participants, aged care clients, families, carers, support coordinators, care partners, allied health professionals and other service providers to ensure supports are coordinated, responsive and centred around the individual.

Our commitment is to provide services that are safe, respectful, reliable and focused on meaningful outcomes.

Care Team

At Ongoing Care, we are proud to have a diverse, experienced and qualified multidisciplinary team supporting the delivery of our NDIS and Aged Care services.

Our team brings together a broad range of professional qualifications, skills and experience across healthcare, disability, aged care, allied health, public health, biomedical science, nursing, psychology, occupational therapy, physiotherapy, human resources and business administration.

Across our team, qualifications and professional backgrounds include Master’s and Bachelor’s degrees, Public Health, Biomedical Science, Nursing, Physiotherapy, Psychology and Human Resources, alongside team members currently undertaking further university studies, including Occupational Therapy and other health-related disciplines.

Our support workers hold relevant qualifications in Individual Support and/or Disability and Aged Care, together with required worker screening, induction and ongoing training relevant to their roles.

Our administration team also brings healthcare knowledge and professional experience, including Registered Nursing, helping strengthen our understanding of quality, safety and the individual needs of the people we support.

Our Human Resources function is managed by appropriately qualified personnel, supporting recruitment, worker compliance, performance, professional development and workforce practices.

This combination of professional knowledge and practical experience allows our team to work collaboratively with participants, aged care clients, families, carers and other professionals.

We are committed to maintaining a capable workforce through careful recruitment, appropriate qualifications, worker screening, mandatory training, supervision and ongoing professional development.
